GENERAL RELEASE LIABILITY

I waive all claims against any and all persons associated with Eastside Boxing Club, LLC. I understand that I am participating in an activity, which involves body contact. I understand that boxing with or without contact and other activities that are excessively physical and may cause excessive pain. Under the terms of this “Release” and sufficiency of which is hereby acknowledged, the undersigned hereby releases and forever discharges the Eastside Boxing Club of “Liability”. In consideration of the acceptance of my application as a participant to Eastside Boxing Club I hereby agree to assume all risks attendant upon myself while participating in this club. I hereby waive, release, and discharge any and all claims for death, personal injury, or property damage which I may have, or which may hereafter accrue to me, as a result of my participation in Eastside Boxing Club. I agree to indemnify and hold harmless Eastside Boxing Club and its members chapters and/or any of their agents, servants, volunteers, or employees by reason of any accident, death, injury, or damages, to persons or property which I may suffer, while participating in the. This release is intended to discharge in advance the Eastside Boxing Club its member
chapters and/or any of their agents, servants, or employees by any reason of any accident, death, injury, or damages to persons or property which I may suffer, from and against any and all liability arising out of or connected in any way with my participation organized by the Eastside Boxing Club even though that liability may arise out of negligence or carelessness on the part of the persons or entities mentioned above.
